Question about I-693

My wife (from Japan) and I are compiling the things we need for her I-485. As it turns out, she doesn't have her I-693 with her. All she has is an X-ray from her doctor in Japan. When I asked her about it, she said that the people at the embassy in Japan took all that stuff from her, and told her that she just needed the X-ray. What should we do?

Did she get the vaccination shots in Japan? If so, does she have a copy of her DS-3025 vaccination form? If so, she only has to go to a civil surgeon to get the information from the DS-3025 transferred to the I-693A vaccination supplement. Check out the EWOK pinned thread at the top of this forum for more information on that.

Yes, you need to fill out the I-864 form. You filled out the I-134 form for the K1 visa. That document is not binding, the I-864 document is! That's the one they want for the AOS.

Try to obtain the documents from the doctor in Japan and then make an appointment with a civil surgeon here. To locate one: https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV
