My fiance and I have both had more than two jobs and more than two addresses in the past five years. How do I include these in Part 8 (Additional Information)? Does anyone have an example? And whose name do I put at the top--mine (the petitioner's) or his (the beneficiary's)?

 

Also, do I have to include each address I lived in while in college (I recently graduated so it falls within the past 5 years)? Throughout college, I always listed my permanent address on paperwork as my family house I grew up in, so can I JUST list that one?
