Insurance evidence for I-944

Not sure where to ask this question, so if incorrect I apologize.

 

I know I need a copy of the policy with names of who is covered on it, with start and end dates.

 

I am covered under my husband's work insurance.

 

Where do I get that? I found on my insurances website a 154 paged PDF of every single detail my coverage, but it doesn't have names or dates on it. I doubt they want that, they prob just want a summary?

 

The website has the dates, but unless I print out the home page with those dates I'm not sure where I'd get it.

 

It also has the coverages on the main page but it's one of those clickable menus that sorts by type and says what's covered under that type, so unless I screenshot each different section on that page...

 

But it also doesn't say who is all covered. Shows my name, but doesn't even mention my husband anywhere.

 

Am I looking in the wrong place? Should I just email the Insurance people of what I need?

 

Sorry if this doesn't make sense, it's just this annoying form.

I do. But the insurance card doesn't have the dates on it, and I think in the instructions somewhere they say to not send in just that.

try calling the insurance provider and ask them to issue a letter, with their letterhead and stuff, stating that you are covered under whatever plan, including the coverage date and everything else the i-944 instructions ask you to provide evidence for. also, ask your husband to get his company's hr to provide a letter stating his insurance coverage, who's covered, plan, dates and etc

you're right, the instructions mention that the insurance card is not enough to prove the stuff they want you to prove...

I did something very similar! I’m adjusting from an F-1, so my coverage is through my school. I reached out to the insurance liaison and had them write a letter on the school’s letterhead confirming that I was covered under the plan.  On top of that, I included a PDF with the details of the coverage from the insurance company itself, a screenshot from the online user website that shows my name and that I’m currently covered, and a copy of my insurance card.

09/25/20: AOS package (I-130/I-485/I-765/I-944) mailed out via USPS.

09/27/20: Package received by USPS.

10/24/20: Received NOA1 in the mail. PD is 09/28/20.

01/05/21: Biometrics appointment completed (Brooklyn ASC). Case updated to show fingerprints were taken (I-486 & I-765).

01/05/21: I-131 mailed via UPS.

01/17/21: Received IOE receipt number for I-131 via text.

01/25/21: Received a biometrics reuse notice for I-131.

02/02/21: I-485 Case is ready to be scheduled for an interview.

02/04/21: Received I-693 deficiency notice in the mail (dated 01/29/21).

02/09/21: Advance Parole document was produced.

02/16/21: AP in hand.
02/26/21: New Card Is Being Produced (EAD).

03/04/21: EAD in hand.

09/01/21: EAD renewal paperwork received by USCIS. IOE receipt number.

09/07/21: Receipt / 180-day automatic extension received in the mail.

09/09/21: Received a biometrics reuse notice for I-765 renewal.

10/25/21: AP renewal NOA and biometrics reuse received in the mail.

11/04/21: Interview was scheduled for 12/02.
