I noticed there was a new affidavit of support form today. It says in the instructions that we must submit the bank statement from an officer and letter from employer. So does this mean they don't look at the tax returns anymore? What about paystubs? If we submit tax returns and paystubs only, we will be denied then?

It appears they look at tax returns if the sponsor is self employed.  As to the I134 being a new form, the available form on the USCIS website is dated February 2019, and the instructions PDF are also from February 2019, so I am not sure what has changed.

Oh youre right. But the tokyo embassy website says we can submit other evidence and we only need two. If i cannot obtain the letter from my employer, they said they can only give me my job offer form, i can use tax returns and paystubs? The embassy and uscis website says different things.

You follow what the embassy says. You’re now dealing with DOS, not USCIS.

Awesome. So i wonder if tax return, recent bank statement, and paystubs will be good.

What is good is interpreting and following consulate instructions

YMMV

It was my mistake. The current one is good!

Have you done the medical appointment? How much did it cost? 😱 And what evidence did you show for the i134?

But I saw the form is expiring on 28 Feb 2021😳

I have my medical appt this week.

The cost will be vary it depends on how many vaccines you have to take. There is 2 diffarent medical facilities in Tokyo for VISA and the Seibo is the one I am going for my medical.

My interview will be in March, we are going to use letter from employer, bank statement and pay stubs or tax transcripts.

In addition would you mind complete your "Timeline"?.

You are looking at the wrong place on the form. The top left corner does have wording of OMB expires 2/28/21. That is NOT the expiration date of the form. It is the expiration date for comments on the form.  FYI- OMB is the office of management and budget. They are the ones who review  government forms to try to make sure they are effective and simple. So when a form is updated they have an open comment period where they take complaints and suggestions and track issues people may be having with the form.

You need to look at the bottom left corner on the forms you are using for the edition date. The 134 found at USCIS website says 2/13/19 on the bottom corner. On the main USCIS page where you download forms it has drop down boxes- one says 'edition date'

"Edition Date
02/13/19. You can find the edition date at the bottom of the page on the form and instructions.

Dates are listed in mm/dd/yy format."

So you want to check your forms by looking at the bottom corner and making sure it matches the edition date posted.
