You don't pay for the green card at USEM.  If you mean the immigrant fee to produce the plastic green card, it's USD 220 and you pay that to USCIS online after getting the visa.  Details here -- https://my.uscis.gov/uscis-immigrant-fee/

It's PhP 17,025 for adults.  Can be paid via cash or credit card at SLEC or through Security Bank.

Yes, negative COVID PCR test results are now required to travel to the US, but travelers may get tested at any reputable clinic/lab of their choice.  The testing facility at PAL Learning Center is often mentioned and they charge less than SLEC for the PCR test.
