This isn't mentioned any the list of forms to send in with the I-485, but it came with the paperwork from USCIS. So do we "have" to send this along with the I-485 form? thanks... :)

Do you mean the G325-A Biographic data sheet? If so then yes, it needs to be sent. I don't know what else you might be talking about. What sort of questions does it have on it?

No, it's not the Biographic data sheet. It came attached to the I-485 form. It says "FORM I-485 DATE SHEET" on the top. Under that it says "Indianapolis". It says "Please print in block letters or you will have to redo this Date Sheet when your application is submitted for processing". They have him fill out his name, address, sex, DOB, City of birth, country of birth, country of nationality, country of last residence, marital status, asks if he's employed, occupation, Nonimmigration classification at time of application filing, Mothers first name, fathers first name, U.S. Consulate or Embassy issuing visa, Date of issue, Number of last nonimmigration visa, classification of last nonimmigration visa. That's all. We're sending the paperwork off today so I'll just mail it with.

The USCIS is a large organization - please be more specific as to how you ordered this paperwork and where it came to you from. Was it the Indianapolis Sub Office?

YEs it is the same info asked on the I-485.

If I remember correctly, I requested the forms online.

Either way, I sent it in with I-485 just to be safe.
