K1 AOS - Medical Examination & Sealed Envelope

Hello,

I am about to submit the AOS package for my wife and had one concern regarding the Medical Examination & Vaccination Record.

Is the DS-3025 that was previously completed as part of the K1 Visa requirement enough ? (Performed less than a year ago). 

 

When my wife entered the US, she had two envelopes. One envelope was taken by the officer (the big envelope), and the other was returned to her. The envelope that was returned, has a "DO NOT OPEN" stamp and couple of stickesr. This envelope is clearly from the Hospital where she performed all her vaccinations / examinations.

 

What to do with envelope? I did not open it, and I am unsure if I should submit that along with I 485 package ?
