I'm a green card holder. My wife's  interviewd 12/3/2020 at USA embassy Colombo Sri Lanka. Same day she got a congregation letter from embassy. But after the interview her case was changed into Refused. Still passport is holding from embassy. Is there anyone has same experience in Sri Lanka embassy? If I send extreme hardship letter to embassy, can her passport take back with visa? Please help me.

I would search the forum for your country specifically, refused status is common and usually not a bad thing.

Those aren't subject to it

GC Holder filing for spouse is under that.  What category did he file for spouse? Just because she is a physician does not mean that the embassy will honor her request for exemption.  It will most likely be issued after March 31st.
