The IO will see the original DS-3015, which is marked complete. It will be in the sealed envelope. It really sounds like you have no need to worry.

Thank you! Hopefully it will be a smooth sail when im applying for AOS.

 

But will still double check with my panel physician tomorrow and ask them if they have another copy of my original ds-3025 just in case. and asked them about what's the deal with tempering the current copy I have haha 😑

Having the original DS-3025, Vaccination Documentation Worksheet marked COMPLETE is all you need. You are now trying to correct a document that already is correct. Why do more than do will do? Keep your copies safe in the event you may need them down the road. It's my inclination you are fine for the AOS with the DS-3025, Vaccination Documentation Worksheet that is in your embassy packet (original). You're overthinking this process and that's where you get into trouble. Thankfully that you have a community here that has opined in the hopes of helping you. We all have come to the same conclusion and that is submit the original DS-3025, Vaccination Documentation Worksheet and all will be ok.

If a vaccination requires a series of doses, and if at the time of the medical you only needed to have the first dose of the series,  you are still good to go.

There is no submitting the ds3025 as a part of your application.   They will only review the one in the packet you provided the POE.
