Hi Everyone! 
I filed my daughter’s petition on August 2016. I received a letter requesting adtional evidence about my name. When I got married the first time, I did not change my single name, never used a “married” name. However, on my daughter’s birth certificate shows my name with my ex-husband last name. I do not have any document neither IDs with my ex-husband’s last name. I have the marriage certificate showing my name as single, and the divorce decree. What other documents can I send as a proof that I am the same person and I am my daughter’s mother? Please I need your advices!!! Thanks!!

You could try to write an affidavit that they put your husband's last name on your daughter's birth certificate as a clerical error and you never left your maiden name.

 

Another option would be a DNA test.

Contradictions without citations only make you look dumb.

Thank you Demise. I also have the marriage certificate for my first marriage and the divorce decree. I believe it will work as proof that I am the same person. I will do the affidavit that you recommend. Will it help if I send 2 more affidavits from people who know me and know that I never changed my name?
