For my civil documents with translation I listed down the documents in the file in the comments ( birth cert & translation ), etc 

 

while for additional evidence I commented that the pdf file consisted of pictures from the times we met/spent time together. 

and if they requested any additional documents I’ll add any comments / put a note why I’m uploading the document. 

 

or else I’ll just leave it empty for most of it.

“Please refer to all (number of) pages.” Have put that on documents that have multiple pages. It might be a no-brainer but I myself have made the error of checking only the first page of documents, especially if it can be a standalone one.

I used comments as like a table of contents, saying what is going to be on every page (especially for docs that were stitched together in a PDF but didn't have a cover letter). I phrased it loosely in the form of a letter and I thanked them for their time and effort at the bottom of the comment.
