EAD renewal

Hello everyone! 

I am currently on AOS but the green card process is taking a long time unfortunately. And because of all that my driving licence expires in June 2021 (the date when my EAD expires too). 

I’m planning on applying for a new EAD in the beginning of January, and I have a couple of questions: 

1) do I have to pay for a new EAD card? Or I can just submit the receipt from my green card. 

2) do I just submit the same form as I did the first time for the employment authorization document?
