I filed IV petition for my wife on mid of 2018, the Interview was scheduled at Islamabad, Pakistan on December 2019, after the interview they issued form 221(g) and asked my wife to submit my previous marriage termination certificate, which was submitted through courier next day, after that, thru various messages to Embassy, we were told that the document received by the Embassy and they are still reviewing it, and due to Covid the Embassy processing is on hold, after an year, the status on the case is changed to "Return to NVC" without any notification. Anyone knows whats going on? and whats the next step?

I think they might have to review and reschedule your interview.  You should honestly contact NVC, I recommend you try contacting them between 11PM-12AM since they are open from 7AM to midnight.  I was able to get in touch with a customer service rep within 2 minutes every time during the later hours.
