Hardship waivers are processed by USCIS. USCIS does not issue visas so to answer your question no a visa is not automatically issued. You still have to go through consular processing. Keep in mind administrative processing for Pakistan takes longer than many other countries and a hardship waiver will not expedite this step of the process.

If he's now A Canadian citizen and he's been out of Pakistan for over 12 years besides for short visits will they still hold things. 

Yeah will run the required background checks no matter where he lives now or what embassy he interviews at. The general rule of thumb is that those who have lived in Pakistan (especially as adults) undergo a lengthy security check
