Are R1 visas banned currently? Is Your embassy open for appointments at this time? Those are two factors. Unfortunately if you’ve been denied twice I don’t think you will qualify for an expedited appointment but I appreciate your work.

Then that is your answer. Nothing you can do about that. If they are not doing nonimmigrant visas then you can't force them to. Be patient. Your time will come.

R1 is a religious visa, not medical.

 

And amusingly the one most connected with fraud.

Immigration journey is not: fast, for the faint at heart, easy, cheap, for the impatient nor right away. If more than 50% of this applies to you, best get off the bus.

OP, are you trying to say that you're urgently needed as a religious worker in a hospital?  If so, I have to say that the vast majority of US hospitals are secular, and while all of them have chaplaincy services, it is usually a very small staff.  

 

'Urgent need' that would compel a US embassy to expedite an interview would more than likely to be for front-line COVID-19 (ICU) physicians and RNs.  Those workers are the ones who are in shortage here in the US, in this pandemic.  Chaplains as a labor group are not in a shortage.
