Because visas are never approved during the interview.  All visas go through some administrative process and checks after the interview before final approval.  This process can take a long time for some cases and countries.

Her first interview was aug 2019. on the second interview did the embassy have you make the 221G follow up appointment by your self? or did they make an appoinment for you to come see them? and did they do anything else other then just ask you questions, like take your finger prints or ask you which address will you like to get the visa when its ready?

My husband had second interview in July, and submitted new medical and passport. Today he received a call wanting a secondary and primary phone number and was told his documents are being given to the officer and our case changed from refused to administrative processing. Is the wait finally over for us? Any ideas?
