Answered No and said that it was lost because we did look for it. 

But we have since found it. 

DS-260 hasn't been reviewed yet as we haven't submitted all docs yet. 

Anything we should do or upload with civil docs?

Immunization record should be a part of the medical exam report

Yes but the DS-260 asks about it.  I'm guessing as long as we bring it to the exam it's fine.

Correct.  It's not an issue for the DS260.

Facts are cheap...knowing how to use them is precious...
