Evidence of legal name change

Hello!

My future husband has started filing I-129f petition. So it also requires evidence of legal name change. His last name was changed when he was adopted. So my question is what is the evidence in this situation?  Also I wanted to know if this I-129f requires affidavit of support or not.

So affidavit is only when petition will be approved (if it even will be)  and I will go to embassy?

Typically the applicant hand carries it in to the interview

Yes it is, assuming the petitioner was born in the US.
