As far as I can remember it did have parental information on it - I'm actually just trying to call him and ask because I can't recall all too well as it was a stressful process compiling it all! Nope, I wanted to but we weren't able to - otherwise that would've negated any worries haha

I didn't do K1, but with my docs I sent copy of short form birth certificated and copy of passport info page since I was sending passport stamps anyway and that was accepted.
