My spouse has been issued a B2 a couple of years ago. In filling out DS-260 for my spouse's CR1, we came across the question asking about other names ever used. This question was answered "no" in the DS-160 during her B2 interview, but has been answered "yes" during the I-130 stage, and will also be answered "yes" now at the DS-260 stage. It was an honest mistake that my spouse forgot, as it was a last name that was modified to include an adoptive parent's last name back when my spouse was a kid. Is this discrepancy an issue? Could this be considered material? So worried about this, any advise greatly appreciated!

It does not sound material,  maybe a minor glitch.
