Hello,

 

I have been in the UK for the past month and half, trying to get an appointment with the London embassy to file for my son's CRBA. Despite checking everyday, I have failed to get an appointment with them. 

I am a USC, naturalized, fulfilling the physical presence requirement. I am leaving for US in two days, and was wondering what documents I should leave here in the UK. So far:

CRBA application, proof of physical presence (all my university transcripts totaling over 8 years, employment verification letters), my naturalization certificate (original).

What I intend to send from the US: notarized DS3053. 

 

My wife will be attending the appointment (once we get it), with the baby. Anything else that I'm missing? Do I also need to send my passport over once I'm back in the US? Do I have to get anything notarized here locally in the UK?

Also -- since we were married at the time of baby's birth, I do not need DS5507 correct?

If one custodial parent is not able to attend, s/he must submit a notarized Form DS-3053 Statement of Consent. The form can be downloaded here (PDF).  At the time the DS-3053 form is presented, a copy of the same identity document used to notarize the form must also be provided.

 

If the parents were not married at the time of the child’s birth and the U.S. citizen father is not able to attend the appointment, Form DS-5507 Affidavit of Parentage, Physical Presence and Support is required.  The form can be downloaded here (PDF). The form must be completed, signed and notarized, and should be presented along with a copy of the same identity document used to notarize the form.
