I need an info.

Can I get married in us then fly back and apply for CR1 while in Italy or I have to get married outside the US?

Sure.  All you need to do is get  married.  Marry anywhere you want.

You will want some proof that you intend on returning home, and don't lie to CBP.

Yeah and I will return home then apply for the visa. Is that possible?

No reason not to.   You are leaving the US before 90 days and interviewing in you home country.

Possible and recommended.  USCIS sometimes gets confused if you file i130 when you are in the USA,  they would not transfer the case to the NVC for embassy processing.

I was just confused and worried cause I thought it was forbidden to get married while on esta..

Perfectly fine to marry and then leave.  Not fine to show up at the US airport intending to marry, stay and adjust status.  Make sure to have a return ticket and proof of your ties to home.

Yes, but you don't start the process.  The process starts with the US Citizen filing a petition in your behalf.  When that petition is approved, you are invited to apply for a visa.
