DS-230 (Other name used or aliases (If married woman, give maiden name)

Question about DS-230

My wife filled Ds-230 in line 2 and 27(other names used or aliases (if married woman given madien name)

She put her name (first name and middle name). because her first name, middle name, lastname is what on passport but it differes from

her birthcertificate. on her birthcertificate she dont have her last name. She has only first and middle name. Is that problem?

Do we need some supporting evidence or document to proof that its her birth certificate?

Example in case its confusing

let sayu her first name is : Gita

her middle name is: Kaur

her last name is: Sidhu

So her full name is Gita kaur Sidhu

Her birth certificate has: Gita Kaur (first and middle only)

her passport contains: Gita Kaur sidhu (first middle and last)

now on ds-230 should we put in line 3 and 27 (Gita kaur) because its different then her passport

and she used it on her brith certificate?????? or we should leave it blank or N/A

Now we have showen on her I-130 and G-325 biographi forum. that she has no other name used. its N/A in those forums

will this cause any conflict or problem? please suggest us
