You don't pay for any affidavits, I have no idea what you're talking about there. DS-160 is a per-requisite to receiving Packet 4, but wait until you get Packet 3 to submit it. You're free to fill out the DS-160 before receiving Packet 3, and just submit it as soon as you get it.

I got packet 3 instructions on March but submitted on June but I’m still waiting for packet 4

Ok, there's nothing else you can do right now. There's a visa fee you have to pay but only after you're able to schedule an interview/have an interview scheduled when you receive packet 4.

New interview scheduled this morning for November 4.

I have not received P4 though, so I'm not sure how to go about scheduling a medical, but I will be phoning Dr. Cheema's office by Friday to inquire about new protocols in place due to COVID-19.
