Okay, so filling out forms for adjust of status and they are looking for a medical to be done. I had my medical done jus tover a month ago in London. The website states i need to fill in I693A, but I cannot find this online. The medical place in London did not give me an !-693A. What do I do? Do I need to fill in an I693 and go for a medical exam before i can submit my forms?

You need to go see a civil surgeon for that form, the doctor will transfer your vaccination record to that form. Take a copy of I-485 instructions with you to point at the part where K-vis only needs suplement, or they will insist on a fill medical for much more $$ than just the supplement.

You shouldn't need an entire medical exam done again. You will just need the sheet with the vaccinations and other medical test information transcribed to the I693A form. Here is a link to the form: http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf. Download the form and have take it to a Civil Surgeon in your area for the transcription. Now - all the medical place gave me in London was a sheet with vaccination information - is it enough for me to take that along, because that's all I have...

take this vaccination information from london to a civil surgeon and let it transfer to the form you need.

The sheet that you are talking about, does it have your prior vacinations and dates listed on it? If it does, then yes take that with you. Shop around for the civil surgeon until you find one that will do what you need and NOT what they want. DO NOT let them do another complete physical! Good luck... :thumbs:

No longer in the hands of the USCIS, for my wife at least, next up MIL.

I did the same and it all went okay, the CS and the secretary looked at my passport too. Make sure you get a copy of I-693 Vacc. Supplement transcribed for your records to keep.
