No DS-3025

Hi,

While filling out the forms for AOS we realized that we didn't get a DS-3025 form. We don't have any proof of what vaccines my fiance has had. How can we get this to give to the civil surgeon to fill out the I-693A? Can the embassy give us a copy if they have one? Should we just go ahead and get a blood test to see what vaccines we need?

i did not get this form from my medical either. What I did was sending them the copy of my entire vaccination record book (not sure if you do have that or not. It is a small booklet that one (at least in Germany) got issued when you were born that keeps track of all your vaccinations). If yes, copy that and sent it to them.

That's the problem, we can't find the vaccination booklet.

You can ask the panel doctor in your home country to mail or fax you the filled out vaccination worksheet which is the DS3025 and then get it transfered to the I693 by a civil surgeon here.

Or you can get a blood titre test to find out if you need additional vaccinations, however that will be very expensive.

You can also get the vaccinations anywhere else and take the record you receive to the civil surgeon to tranfer.

It would be all around cheaper to try to determine from the panel doctor which ones you already had.
