Hi all! I've become a US citizen in February of this year and I also applied for a name change during that time. I've been trying to get a new SSN card but they keep sending my form back saying I need the actual document that says I changed my name and it was approved. I don't remember what this document looks like and I can't find any in my documents folder from USCIS. I have my naturalization certificate but not the name change document. For all naturalized citizen who also changed their name, do you know what this document looks like and where can I find a replacement?

Well the first you'd need to do is contact the district court that administered your oath and see if they have a record of your name change petition in their system. This is sent to the court by USCIS before you're supposed to take your oath and so if they can't find a record of it chances are that USCIS forgot to send it over. This happened to me and I spent weeks trying to get USCIS to send the petition over to the court and had to raise several service requests. Hope that helps!

Yes! Normally USCIS will staple your naturalization certificate with the name change document during your oath. Btw, the name change form is part of the N400 application you filled out and you sign it during the interview and it basically shows your old name and your new name.

This is correct. I have updated my SSN and they need you to mail in both your naturalization certificate as well as the name change document. Even the DMV or banks won't process your name change without this document and so what you need to do is call USCIS and raise a service ticket and pray someone competent handles it. The field office will contact you once the service request reaches them and they will simply send the petition to the court and mail you a certified copy.

I've been calling USCIS all morning and all I get is an automated thing that keeps going in circle. I'm so frustrated! Thank you for your help I guess I just need to keep calling.

Was your oath done by a Judge?

 

If USCIS did the oath, your new name will be on the form, but its not official.

You should have done the oath in a court by a judge.

This thread has me a bit spooked. Because while I didn’t exactly change my name, I lost my middle name on naturalization because my middle name wasn’t on my birth certificate. The IO told me if I wanted to keep the middle name I’d have to have a court oath with an official name change and I chose not to do that. I’m wondering if social security is going to give me a hard time when I attempt to change my name with them? 
 

edit - I’m still same first and last name. Just no middle name.

My brother got his lost name change document back from the Federal court. You may give it a try.

It is called N-662 form (Petition for name change document).  When you call USCIS, tell them that you did not receive your N-662 form during naturalization (if you haven't received ONE) so that they can raise a service request or help you to get N-662 form. 

 

If you lost it, then you have to request a certified copy of the N-662 form and was told it will be given by district court if they have it in their records.

 

I was told sometimes it will be in the USCIS envelop they give us during oath ceremony.

UPDATE: USCIS said I shouldn't need it for SSN. So I called my local SSN office and they said I can just sent in again my naturalization certificate and a form of ID. I received my letter back and I'm now expecting my new SSN with my new name. My concern now is should I still try to get my actual name change form? In case I may need it in the future?
