I applied for my N400, since I have been confined in the mental hospital voluntarily, what kind of documents should I bring for the interview regarding my admission to the mental institution?

Yes but that is a different question asked. Now I am asking what kind of documents to bring regarding my confinement in the mental institution? I am clueless. Maybe someone here has the same experience as me.

Did the hospital not provide a discharge summary when you left?

Discharge or after visit summary. If you tossed it you can call the hospitals medical records department for a copy. Or if your health system has an app you should be able to print your discharge summary from there.

Apply for Social Security Card: 12/30/14 Connecticut

Marriage: 1/26/15

Thank you, that’s the word I was looking for. Discharge summary and medical records. I will request for it, thank you! More power!

I passed US citizenship test. I brought medical records with me. I explain that I answered yes on the question from N400 regarding mental institute. Everything went well, didn’t affect my citizenship interview at all
