Immigrant visas are limited to the validity of the medical examination for a maximum of six months.  Centers for Disease Control and Prevention approved a one-month extension for medical examinations conducted between January 1, 2020, and June 30, 2020.  If you were unable to travel on your issued visa or obtained your medical examination but did not receive your visa, contact the Immigrant Visa Unit of the U.S. Embassy or Consulate that issued or is adjudicating your visa application to determine whether you may be issued or reissued a visa for one additional month.  If you are not able to travel within the one additional month, consider waiting until you are able to travel and obtain a new, full validity medical examination and visa.

Can you some shed some light on this?

Correct. DOS follows the medical guidance of the CDC, and the CDC granted a 1 month extension of medical exams conducting from January 1st through June 30th.

The medical exam normally expires within 6 months, so if you had a visa medical conducted in early February 2020 and valid until July 2020, it would now be valid until the same day in August 2020.

So do we have to contact the Embassy for the extension?

The instructions state to contact the consulate to have it extended. Although, unless they already issued the visa or are ready to issue it, there is little point in contacting them yet IMHO.

My husband went for his second medical in May 19 and was told the visa will be issued after 4 weeks and we haven’t heard anything because the visa section is closed in the Embassy due to Covid closures back in March... so I’m wondering if we should contact the embassy or not

I read It as We only contact embassy if You had interview done and your medical was taken during those time frame & you still don’t have your visa issued. Because issued visa is based in medical expiration

if we all were suppose to email embassy, for ppl who had their interviews cancelled (after taking medical) then it would be too many ppl emailing

I think extension is auto granted

That would make sense. But it says:

"If you were unable to travel on your issued visa or obtained your medical examination but did not receive your visa, contact the Immigrant Visa Unit of the U.S. Embassy or Consulate that issued or is adjudicating your visa application to determine whether you may be issued or reissued a visa for one additional month."

The points in bold only apply to a visa that was not yet issued.

But yes, if you haven't interviewed yet, then I would not pursue anything. They can't issue the visa w/o the interview anyway.

At which point in the process are K1s considered an immigration visa? I know they're non-immigrant with INTENT to immigrate, but I also read that there's a point it's considered immigrant. I'm wondering if this extension would apply to me. I had a valid visa but was unable to travel due to the bans. My visa has now expired, as has my medical.

Apologies in advance if this is already answered elsewhere.

K-1 is always only non-immigrant.

It's handled by the immigrant visa unit at the embassy. The extension here presumably would apply to a K-1 just like any other visa that requires a medical from a panel physician.
