I was never given a vaccination sheet at my medical and i have just been sent a letter asking for I-693A??? I only have the list of vaccinations my dr from home gave me plus 2 others i had to get after i had medical signed by my own dr is ok to show this to a civil surgeon to transfer?? Also anyone reccomend a cheap civil surgeon we dont have a lot of money

You take that list of vaccinations from your doctor back home, to a US Civil Surgeon and the US doctor will transfer them to a vaccination supplement.

I recomend bringing a copy of the I-485 instructions with you to the doctor so you can point at the part that only a supplement is needed for K-visa holders, along with the RFE. They wont insist on a full medical and the fee to do the supplement is $20-$40 typically.

You need to show proof of vaccinations - I don't think you HAVE to have the suppliment from the medical. I just had letters from the Public Health office and the travel clinics from Canada with the date and shot given - the Civil Surgeon that accepted that as proof.

Do some digging - I read somewhere about a Civil Surgeon that you could mail your information to (texas maybe).. you should be able to find it with the search feature (or someone else may have the info more close at hand).

2007-05-21: I485 Approved

2007-05-24: EAD Card Received

2009-02-27: Sent I-751

2009-03-07: NOA I-751

2009-03-31: Biometrics Appt. Hartford

2009-07-21: Touched (first time since biometrics) Perhaps address change?

2009-07-28: Approved at VSC

2009-08-25: Received card in the mail

Naturalization

2012-08-20: Submitted N-400

2013-01-18: Became Citizen
