EAD/AP Renewal question

Hi, 

 

I hope everyone is staying safe!

 

I'm adjusting status from i485 and filing for my EAD/AP renewal, I had a couple of questions and need confirmation that the following list of documents is correct:

 

Renewal under C9 Category:
- Signed I-765 (EAD) application
- Signed I-131 (AP) application
- A copy of EAD/AP (front & back)
- 4 2"x2" passport-style photographs
- I-797C (NOA1) for I-485 (AOS) application
- I-797C (NOA2) for I-765 (EAD)
- I-797C (NOA2) for I-131 (AP)
- Cut-off portion from combo card
- G-1145 (E-Notification of Application/Petition Acceptance)
- A copy of passport
- I-94 

 

Additionally, if someone could please answer the following questions:

 

1) I entered the US using my AP in 2019 December and my class of admission in my I94 is "DA" what should I write on the question 24 and question 25 below on the I765 form:

The form instructions do say to include those. I'll just note that for my first C9 EAD renewal (filed Dec. 2018 and approved Sep. 2019) I didn't include them and I had no issues. I recently filed my second C9 renewal (filed July 2020) without them and the lockbox accepted the filing and sent me the 2 receipt notices. In both experiences they sent me a "Biometric Reuse" notice: "U. S. Citizenship and Immigration Services (USCIS) has received your form and is currently processing your application, petition, or request. This notice informs you that USCIS is able to reuse your previously captured fingerprints and other biometrics. USCIS will run the same security checks and use your biometric data as in the past, however, it is not necessary for you to appear at a USCIS Application Support Center (ASC) for a biometrics appointment."

The I-765 packet should have it's own set of documents and the I-131 packet should have it's own set of documents. Both packets will be sent in the same envelope. I am enclosing the following documents in support of my application for Employment Authorization:

  1. Form I-765, Application for Employment Authorization
  2. Form G-1145, E-Notification of Application/Petition Acceptance
  3. Copy of I-797C, Notice of Action, receipt notice of pending I-485
  4. Copy of my current Employment Authorization Document that expires within 180 days

#24 is "DA" and #25 is "Pending AOS under INA 245"

My class of admission will be "DA" as well, correct?

Additionally, this is renewal do I have to enter yes to the following question? I was never issued a reentry permit but a advance parole so I've answered no.
