This may information that is already available here on VJ, but I'm having trouble finding it. My fiance just picked up his medical exam forms from the doctor....she gave them to him in a sealed envelope. He was not given a copy for his records. How do we obtain such a copy...does the consulate return the medical forms once they open and review? I am just worried...if the consulate does not return these forms to my fiance and we are not able to get a copy, we may have to get a completely new exam done here in the states before we can file for AOS.

Any experience with this anyone? Advice? :help:

Don't worry, they will forward the medical results to the USCIS in that mysterious brown envelope.

All you need is vaccination record, either from foreign country, of get vaccinated in the USA after arriving on the K-visa, then take the vaccination record, along with the instructions for I-485 showing that vaccination supplement is all that is needed to a civil surgeon in the USA to transfer to a vaccination supplement. NO need for full medical.

OUR TIME LINE Please do a timeline it helps us all, thanks.

Is now a US Citizen immigration completed Jan 12, 2012.

OH, I see, thanks for clearing that up! :) Still confused about one thing. At the medical exam, the doctor deemed that my fiance should receive 2 vaccinations (as appropriate for him). He was then sent to another place to get these vaccines, but the record (receipt) for them was turned in to the doctor....and my fiance does not now have a copy of that. Should I have him try and request another copy from the place where he was immunized? If not, we have no record of what vaccines he received for to have that transferred to the vaccination supplement. He would then have to get a new set of vaccines again here in the US, which can cost a lot of $$. :unsure:

Thanks for the suggestion Yodrak, but that is what I instructed my SO to do. The doctor said that she cannot give him a copy of the forms she filled out (they had to be in the sealed envelope to be opened by the consulate on the day of the interivew.) I'm not sure why, but I guess this is part of the Hungary specific consular process? So it just remains to see if he can get a copy of what immunization vaccines he was given and bring that....and then have the Vaccination Supplement filled out here by a Civil Surgeon. (We'll cross the "I can't find a Civil Surgeon who will transcribe the vaccination record" problem when we get there....one step at a time. :blush: )

Important information that would have been helpful had it been included in your original post.

In any event, it's universal practice to put the original forms in a sealed envelope but it's common practice to give the visa applicant a photcopy of the DS-3025 vaccination worksheet. The rest of the forms you don't need.

The consulate will put the medical forms in the sealed brown envelope along with the rest of the case documents, to be turned over to CBP on entry to the USA. Perhaps your fiance will be able to get a photocopy of the DS-3025 from the consular officer at the interview if he asks for it, but it's not something to count on.
