Immigration medical

Hi,

I came here under a B2, so I didn't have a physical in France, I did it here in the US yesterday with a local Civil Surgeon.

So the Paper says: Immigration and Naturalization Physical.

Checked on the list, with the fees are:

Immigration and Naturalization Physical

HIV test-INS

Syphylis Serology RPR

TB test

Venipuncture

They didn't check Completion of 1693 supplemental form, but I know he filled a paper about my vaccines (I didn't need any new one) with the dates I had them, the dates I was tested for them etc. Is that the supplemental form?

Does it just mean that they didn't charge me for it as I paid for the full medical, or am I missing something?

I'm asking now because I'm going to get the reading of my TB test done tomorrow, so I need to know if I should ask for something else.

The civil surgeon will know everything that needs to go on the forms, and will fill them in for you. It sounds like you had the normal medical so far from the list you give above.
