Hi VJ'ers! I have an AOS question. IS the DS-3025 (Vaccination Document Worksheet) that K-1 fiance receives in HCM the same as the I-693A for the AOS application? It appears to be the same info, with the doctor's signature.

Any advice from the wise and experienced? :)

Things are going well for Linh and I. Next step in the Driver's License (as well as the AOS and EAD waiting).

They are not the same form. But you may have no problem sending in DS-3025 or you may get and RFE for I-693A. Otherwise you can get a designated civil sergeon to transfer the information for you assuming that your fiancee has all the required vaccinations.

Hi Anh Map,

Not sure if the I-693 is a requirement now, officially, or not. But to solve the question of: should I or shouldn't I, we did get the sup-vac done for $10.

Good luck! and NO, your VAC record in Vietnam is NOT a complete record of Vacination, therefore you need the Sup-Vac I-693 to show them your wife have all the required vac done! Good luck!
