Do I have anything to worry about how my DS-3025 was filled out for the placements of the "A" marks signifying the shots as not being age appropriate?

 

Hib and hepatitis A share the same mark between both types of shots, Polio has one mark for two types of shots, Meningococcal has one mark for two types of shots, and Pneumoccal has two marks for four types of shots.

 

It seems really inconsistent, however there were no issues with it at PoE or for obtaining the K1-Visa.

Vaccinations are NOT required for the k1.  POE doesn't review the vaccination form anyway. 

 

Only thing you need to concern yourself with is how is the form marked in the lower left corner.  Was it marked complete?

My concern here isn't for K1 purposes, it is to make sure the form was filled out correctly for AoS.

 

Yeah, in the bottom left corner of the form it was marked as "K Visa applicant voluntarily completed vaccination requirements"

Are you 100% sure about this? This was a quote from the stickied thread about immunizations, which says otherwise, and is why I'm concerned about how it was filled out.

1. Shots marked with a date (at least one of a series)

  • MMR (two are required but if you have "insufficient time interval" Code B in the last column, then you get a waiver)
  • Td or Tdap or DT or DTP or DtaP (One date must be no longer than 10 years ago)
  • Varicella (Either a check by "Vaccine" with a date, OR a check by "Varicella History" if you had chickenpox. History gets you a waiver)
  • Influenza (during flu season)** see note below because you may be okay without it. If it is not flu season, the waiver in the last column is Code F.
  • Everything else marked "not age appropriate" Code A, insufficient time interval B, not flu season F, contraindicated C...things that allow a waiver.

NOTE: If you have shots missing and no waiver for them, stop here and see a Civil Surgeon. "Most of the shots" isn't complete. A note from your home country doctor won't count. All shots must be recorded on the DS-3025 or waived. You will have to take your doctor note to a Civil Surgeon and get him to record it on an official form.

Just completed AOS.  

 

Tdap, mmr and varicella or varicella history is all

My husband had a few that were marked as not age appropriate and he never had a problem with his K1 or AOS. For AOS the civil surgeon did administer the vaccines that were not available in his home country however. For example; in the US generally only old people (65+) get the pneumonia vaccine- they aren't giving you it if you are younger. Now in the future for nonimmigration purposes you may want to know if you are immune to any of these. For example- if you attend college in the US you have to show immunity to certain diseases.
