My wife is getting ready to fill out the family composition sheet and we are not sure what to put in the form.

It states if your parent or spouse has had more then one marriage or concubine, please include.

Do I need to have my wife list my ex-spouse in this form? What if I have no idea of her address what do we do? I have not had contact with her in over 20 years.

Thanks

Yes, any former spouses for each of you are listed. If you don't know her address, I would put "unknown for X years". In my case one was 17 years and my brother disappeared 11 years ago. Just give the information you can.

To make it easy for my wife I created a table for each section with my family's information with all the same fields. I then filled it out and pasted it into an email so she could use it as a guide for my portion.
