When I go here ..

http://www.cfo.gov.ph/emig_manila.htm

it says one of the requirements are ..

Duly completed registration form (downloadable at www.cfo.gov.ph);

When I go there I cannot find the registration form anywhere .. anyone know where I can get a copy? Mylene is going tomorrow and she asked me about it tonight but as you can see I haven't been alot of help as yet.

This isn't the registration form, this is the feedback form to send after the beneficiary is in the US for 2 weeks to 3 months. I am looking for the registration form that she needs to take tomorrow .. Thanks :thumbs:

im not sure but i just went straight to the CFO or Prism and they gave me some form to fill up and have my seminar... i dont think its suppose to be fill before going there??? not sure... never heard of it or tried it... who knows

im not sure but i just went straight to the CFO or Prism and they gave me some form to fill up and have my seminar... i dont think its suppose to be fill before going there??? not sure... never heard of it or tried it... who knows

Hopefully someone knows (as it is stated on the page that it is an option) I'm just not wise enough to locate the form .. Mylene likes to be prepared ahead of time if possible, less things to do at the seminar.

The online forms for the CFO / PRISM in Manila are not the ones they used and are not available online (go figure). I know because I completed both of the forms that could be applicable online for my wife and she took them with her only to discover that those forms are not used. Now, I can not comment on the CFO in Cebu.

Claudeth attended the CFO in Cebu and did the forms there. It was an all-day affair for her and finally got the stamp at about 6:30pm.

I had the forms printed out and accomplish for my CFO seminar but they have a different forms so I end up filling-out their forms for my seminar/counselling (Yellow Certificate). Although the forms in their site and the forms for the stamp and green Certificate are almost Identical, they prefer that the applicant fill-out the forms they have at their office.

03 March 2010 - USCIS Acceptance Confirmation via e-mail and text message

06 March 2010 - received I-797C, Notice of Action/Receipt

01 April 2010 - Biometrics Appointment (Biometrics done 4/7/10)

27 April 2010 - received I-797C/ Request to Appear for Naturalization Interview

02 June 2010 - Interview schedule

17 June 2010 - Oath Taking (Memorial Auditorium, Sacramento, CA)

I downloaded this one and submitted it, no problem.

Edit: I should clarify, this is the form I submitted (I asked if they would accept internet copies and YES they did) only when I had the visa with me, the final steps with CFO.
