I did. That parts that would not let me fill in with the reader, I filled with a black pen.

My credit card was just charged from the Phoenix lockbox (package delivered on June 17)! Hopefully will receive the case number soon.

Good news! Mine was delivered to the Lewisville one last June 16 but haven't received any notifications and my card hasn't been charged yet.

No, I just found out on my own. Tracking shows they just received my packet on Thursday. So I guess I can't do anything besides wait.

No. I did include a copy of my marriage certificate.

I paid using a credit card, using  form g-1450 and i put the total amount of $680 into one form. I would imagine one check would work similarly, but defer to other's actual experience on this.

Adding my timeline: Package was delivered by Fedex to the AZ lockbox location on 6/17/2020. Check has not been cashed yet. 

 

ROC   Fedex shipped package on 6/16/2020

           Package delivered to AZ location on 6/17/2020

           Case received on 6/23/2020 with text update. 

           NOA with 18-month extension received on 7/6/2020.

 

2-year Green card expires on 8/28/2020

Hopefully they will send a RFE for you or may just send the package back. Try not to overthink it. Do you still have enough time within your 90 day window?
