Following! I was planning to translate, even by myself. As you said, it is pretty straightforward. Not sure if the translation should be certified though.

You can send the form to your doctor back home to fill it out, sign it and send it back to you. I do not think you need original. If your doctor does not know English, you can translate it and send another copy for him/her to understand. I am attaching the one I used and It is accepted by every Immigration doctor in Massachusetts. You just need to find a form from your state. Good luck!

I called and asked to expedite and they make the request. You just need to meet the conditions for expedite then call and ask

Looks like its going to be a week for me as well. They were very vague when I asked 'when can I collect it by?' I will probably check again once all blood work comes in....

Oh really....I had to pay $250. However, all the cheaper places did not have appointments for this week. So I had to go with it.  I am blaming myself for waiting this long to get my medical done.

Cool. Unfortunately, my doctor was not ready consider my vaccination from home Country. So I had to do blood work for all the tests.

Blood work does not have anything do to with the vaccines. You need to do 2 things - Blood work (It is 3 tests for diseases) and separate is the vaccination. Whichever you have not had before, they will do it for you. Even you do not have your vaccination report and you go with nothing, they cant make you more than 3 vaccines. After you do the blood for the test and the medical examination ( vaccine and regular check ) the doctor will fill a form called I 693 and will put it in seal envelope that you can not open, you just send it to USCIS. But I asked for a copy so I can see the results from the blood, It costed $10.

Hi. They acted the same with me. But you have to call and check every few days because these people are busy and forgot and do not care basically lol But one of the test ( i think is the Tuberculosis takes so long sometimes )  so If they told you 1 week that is good! You paid 250 for everything ?

I was given Tdap vaccine last year (in US) so they waived me for blood test titer for that Tet antibodies. I took record of MMR, vericella etc. that I received from my home country. All the records were in English. I was made to take blood test titers to verify those vaccinations. I guess it depends on the surgeon that you go to. Thanks for clarifing.

 

Also, did the copy cost you $10 or the whole physical ?

Cool. I will keep it in mind to check regularly.

 

$250 is just for the physical. Blood work is covered by my insurance.
