Dear All, I am planning to submit my J-1 waiver of the two-year home residency requirement. My wife is on a J-2 visa (dependent). Before submitting the DS-3035 online form, I was verifying the information.  It looks like there is an error in the form. The verification page converts my wife's "Surname" to "given name" and "given name" to "surname". Can anyone provide me with some suggestions about what I should do?

Hey Sam, may I ask how did you solve this problem at last? I have the same issue while changing browser, machine etc...None of them works.

That online version is a preview of the one you send over mail. What I did is to make sure the one I sent over mail was right. From what I understand, the online paperwork doesn't start any process, so it should be ok to do that. 

I haven't gotten the waiver yet ( waiting ...) but IMO that is the best choice. From what I remember, the one that shows the name incorrectly is the one that shows it correctly on the mail package. Please check if this is right because I don't really remember.

Thanks buddy. I didnt click 'confirm' since the last name and first name are incorrect on the verification page. I probably should click and continue to see what's going on on that mail package page. Good luck with your waiver!

Hi Sam.. I am facing same issue. How did you overcome this issue? Can you please let me know?
