It’s not mandatory but if you can and it does not cause you too much hassle (some civic surgeon can do physic exam / vaccine / flu shot / std test and give the report back to you within a week), obtaining the medical report and submit it along side with your packet is not a bad idea.

It will be valid for 2 years from the day they receive your packet, then dont have to worry to do it before your interview date. In case of 0.0000001% USCIS decide to waive your interview, they will need you to send in the medical report anw.
