Call and ask for the price to get them to file medical form. It is different from one office to another. 
 

I got mine done in Jackson, Ms.

 

it took me around 3 weeks after I got it done. They need to fill out the paperwork after they get the blood test back

Im currently in California, so can I do my medical examination in California and apply for Adjustment of status in New Orleans? My wife and I are here for couple weeks thats why asking. Would it make uscis suspicious that medicals are done is different state than applying state?

I don’t see that it would be an issue if you have your medical done in California instead of in Louisiana. If asked, I suppose you could explain to the officer what you just told us, i.e. you guys were in California for a few weeks and got it done there. Also, as stated in the USCIS policy manual, you don’t have to include it with your initial application, and wait for the RFE. We submitted it with our initial application to avoid any delays due to RFE. BTW, it took my wife just a few days to get the I-693 done in Baton Rouge.

Im planning to send my medical with my initial application to avoid delays.

It took a few days to get the results back, in Baton Rouge. But I’m not sure if this is the case in other states/locations. Please call a few USCIS-approved doctors nearby and ask them.

Don't do the medical so early in the process. You never know when you'll get an interview and it might not be valid anymore by the time you go for the interview. There's no need to send it with the initial application you'll just bring it to your interview.

No.  Form I-485 Supplement A is not needed for straightforward marriage-based adjustment of status like your case seems to be (based on what I could tell from your few previous posts).

Also on i485 form, one section asks for where I will file for my AOS. It asks for uscis office where I will file it. So it would be “Metairie, LA” in my case right? I live in Metairie. Or it would be closest uscis field office? Please advise.
