Yes I know Visa Journey has copies of those however I was told those are the old forms they are not accepting...? true or not?

:help: I was told I needed these forms for the medical exam in Jordan.... Where do I find these online? Everytime I find a link I get "this page has been moved" but does not tell me where :help:
