Yes to both your questions.Complete photocopies of the all the pages of your passport and front and back of the 1-94

You're filing for AOS, so you don't need all pages of your passport.

From your passport you need a copy of:

*K1 visa and it's 1 time use with immigration stamp

*Biographic page

And the I-94 needs to be a copy of the front and back.
