So she’s using a green card to vacation here while she lives somewhere else? That won’t last long. She can apply for a re-entry permit but what does she have to demonstrate she is a resident here?

Timeline in brief:

Married: September 27, 2014

I-130 filed: February 5, 2016

NOA1: February 8, 2016 Nebraska

NOA2: July 21, 2016

Interview: December 6, 2016 London

POE: December 19, 2016 Las Vegas

N-400 filed: September 30, 2019

Interview: March 22, 2021 Seattle

Oath: March 22, 2021 COVID-style same-day oath

Yes that’s what we got, re entry permit

Once should be sufficient. Any more than that and her greed card will be in jeopardy. Also, by her not living in the US, she is also increasing the risk of her green card being revoked.

Best for her to return or surrender her card.

My mom is currently here in the us. We got exit visa for her couple of yrs ago for 2 yrs. She got back and been here for 6 months now. Is she allowed to get another exit visa? How many times can she apply for it? Can they deny it?

They can always deny

Nothing to stop your mom from applying;  complete the form, include the fee ...  roll the dice.

Only back 6 months after being outside the States for 2 years, now want to be gone 2 years again ...  that would make me go " hmm ... "      Lots to think about

I am not aware of any limitation, with or without a Rentry Permit she needs to show that she is maintaining her Residency in the US.

The question why may be relevant.
