Hi Guys,

My fiance just came in on a K1 fiance visa and we are getting married this month end.I would like to ask a question about the Vaccination supplement.

I know that he would not need to do another medical exam but he got a vaccination supplement from the U.S consulate in his country. I have heard about form I-693A . How does he get this form

Can't he just submit the one he got for the initial visa and attach it to the I-485 form or if we get married, he would still need to go to a Civil surgeon to get this form?

I also have a question about the marraige certificate. If it gets signed by a minister, do we have to send a copy to the County clerk , make photocopies of it and attach it to the 1-485? or we would have to wait for the certified copy of the form?

Generally - 1) You can send a copy of the marriage certificate instead of the original (I chose to send in the original because they were just $2.50 a piece from the county court (where we got married) and we got 3 of them!)

2) You can take your vaccination worksheet (from your medical test for the K1) to a civil surgeon and have him fill out the I693 - (I just mailed in the vaccination worksheet DS3025 and was not asked for the supplement - but some others have been RFEd for it so do what makes you comfortable.)

They will need a certified marriage certificate. This is the one that returns to you and is proof that your marriage has been registered with the county (usually by the Register of Deeds).
