police record from Thailand

Hello, I wonder if anybody has gotten police record from Thailand.

I'm not a Thai citizen, but have lived in Thailand for more than 1 year, so must get a police certificate for CR1.

On the website, http://travel.state.gov/visa/reciprocity/C.../T/Thailand.htm

It tells me to bring items such as:

the original and a copy of his/her passport

identification card

household registration

name change certificate (if any)

marriage and divorce certificate (if any)

I think identifcation card means Thai ID, and I don't have that so I imagine passport will serve instead of that. I am not sure what household registration is, if that is something a foreigner would have also.

Would marriage and divorce certificate apply to foreigners who were married outside of Thailand (=should I get my husband to mail a marriage certificate from the U.S.)?

If there is a person who is not a Thai citizen but happened to get a police certificate from Thailand and can help me out that would be great. Thank you!
