Certificate of Citizenship Date of Issuance

This might sound like a very stupid question, but I'm looking at my Certificate of Citizenship and it has two dates. The date of when I became a citizen and the other date where it says this "Now therefore in pursuance of the authority contained in Section 341 of the Immigration and Nationality Act, this certificate of citizenship issued this day of thirtieth day of October Two Thousand One"

 

The question is asking "On what date was the Certificate issues" or from the paper form "Date of issuance". I'm guessing I would put 10/30/2001 for the answer and not the day when I became a citizen?

Read it literally, they are asking for the ISSUANCE date  .. and as you noted that date is on the certificate.
