You usually need to include the terms of service from the website you met on for proof they are not a marriage broker.

I would work on getting this ASAP in case you get an RFE, but better to have it and take it to the interview in case they ask for it there. You have to wait and see, it could go either way.

I have not sent mine off yet. How and where should I include the terms of service? In additional information? Do I include it in the package and mention it on the cover letter? Thanks

Include the TOS to avoid a possible RFE.

You can mention the attachments on the cover letter. Just print and include them in the filed packet.

Why mention where you met in the first place? No question on the form asks that. I think you get yourself in a bind by providing too much information. Just be simple.
