Last name question and another question

I was married on k1 and now am applying for adjustment of status for my last name do I use my spouses last name us citezen, or my maiden name i assume only use maiden name where it asks for previous names but not sure,

 

and  for evidence can i use the previous evidence i used for the k1 such as plane tickets of when my spouse came to visit me  or does it have to be evidence after the k1 interview  thanks      and lastly if i had my medical exam 6 months ago am  i even required to send in any proof regarding it

Timeline

If you changed your name on the marriage license, then your maiden name will be a previous name now. My wife now didn't change her name to make things easier this way ( not getting new SS card, drivers license, passport, changing names at banks and things). We just did our AOS interview early February, and I sent everything we had from her arrival here, until we filed. I didn't include anything that I sent with the K1. 

Double check the instructions for the 485, and I believe it even says evidence from when you got married until you file. Even if you don't have much, don't worry, they don't expect too much. I brought the rest of the evidence to the interview and they reviewed it there and added it to the file. We had a decent amount to send, but better to get it filed and processing then waiting to gather more evidence, you can do that while waiting for the interview, and depending on office, that can be a year or more.
